De-Escalation of Broad-Spectrum Antibiotics After Relevant Culture Reports in Medical Ward Patients Audit from A Tertiary Care Hospital in South India

Prolonged and improper usage of antibiotics has been shown as a main reason for the emergence multidrug-resistant pathogens.Antimicrobial stewardship (AMS) programs have to reduce rate resistance. De-escalation antimicrobials is an important component AMS. AMS programmes require regular auditing feedback proper implementation continuous improvement. Hence we did this clinical audit with aim estimating de-escalation rates identifying possible reasons non-de-escalation in our centre. Case sheets patients admitted between 1st October 2020 31st December were screened during discharge data non-de-escalation. Cefeprazone-sulbactum (32%) was commonly used empirical monotherapy audited period. after positive culture reposts 28%. Serious illness (36%) found be most common not done nearly 25% eligible instances without justification.
